Forms and Documents

Builders packet / general information  For builders that submit plans/drawing and are required to obtain  permits, we offer a packet of information to use. Hopefully, you will get this placket here, ahead of time, so that you will have a better understanding of how the Fire District's and the City of Raytown's building approval process works.

Plan review and Construction Inspection Application 
The Fire District is a separate entity from the City of Raytown. Application with the City should be submitted at the same time. Please refer to our fee structure document. Click here to view that document.

Hot Work Permit Application    
This hot work permit is required for any temporary operation involving open flames or producing heat and/or sparks. This includes but is not limited to: Brazing, Cutting, Grinding, Soldering, Torch Applied Roofing, and Welding.

Tent Permit Application 
Along with the permit, a checklist is provided for some of the basic regulations governing the operation of Tents and Other Membrane Structures. It is not possible to cover all regulations with a check list. If you have a particular question or concern, please telephone the Raytown Fire Protection District.  This checklist refers to the regulations that govern tents,  canopies, air-supported or other membrane structures.

Business Safety Self Assessment Form
       This form will prepare you for our inspection, which you will need to receive your occupancy permit. It is our goal for you to utilize this document to locate and mitigate fire and life safety hazards, which will intern expedite our inspection process. This form has been developed using the 2000 International Fire Code.

Babysitter information form (MS Word format) (PDF format)
This is a form that should be filled out for the person that is babysitting your child. It has a place for entering all necessary phone contacts and special instructions for their care. Please see our babysitter page for more information.


Home Safety Checklist 
(MS Word format) Similar to the Business Self Assessment Form, this printable form asks for you to do a home inspection in a survey format. This should alert you to take action on potential hazards in your home. Click here for a PDF. version.

Memorandum of Understanding 
This is the current agreement with IAFF Local 1730 and the Raytown Fire District management. The document is in effect between March 1st, 2006 and February 28th, 2009. This is in MS Word format. Click here for a pdf version

Standing Orders and Protocols This Document reflects the current medical treatment delivered by Raytown Fire and Raytown EMS Emergency Medical Technicians and Paramedics. The document is authorized by the Medical Director of both services. Every employee is trained and tested on the latest changes. This is in MSWord format.

Required Fire District Permits    There are several permits that are required by the fire district. This may or may not include additional permits required by the City of Raytown. There are fees for Fire District Permits. Application must be made during normal business hours or you may download and print the application by clicking here. Contact the Fire Marshal's office for specifics about the application process and further explanation of code requirements that involve the processes that require a permit.
